It is reassuring to know that as well as living in your private suite, you have the comfort knowing whenever you need a little extra care, our staff is ready and waiting. Our caring and dedicated staff are on-site 24 hours a day, 365 days a year, ready to assist with everything from dressing, bathing, oral care, general hygiene as well as delivering meals for those that may not be feeling well.

We put the personal care and comfort of our residents above all else. The personal care plan reflects an individual resident’s needs and routines prior to admission, and is based on discussions with the resident, family members, physicians, and other healthcare providers involved in the well-being of the resident.

 

 

Enhanced Assisted Living

 Enhanced Assisted Living

The Home of the Good Shepard Enhanced Assisted Living allows residents to age in place, which contributes significantly to quality of life. We provide care for residents who are chronically chairfast, require the assistance of another person to walk, who are dependent on medical equipment, or have chronic unmanaged incontinence. Enhanced care is also less expensive than skilled care, when paid for privately.

Choosing a memory loss facility is never easy. Along with it comes the realization that your loved one, your parent, spouse, sibling or friend, can no longer safely and comfortably live at home.

At The Home of the Good Shepherd Memory Care Residence, we do everything we can to provide understanding and support. We invite you to come tour our facility and to meet with us to answer your questions. We’re not only committed to serving those with memory loss, but also to provide help to families with thorough information.

The Home of the Good Shepherd dining services are the cornerstone of our steadfast commitment to provide superior, gracious quality of living for all our residents. Our talented culinary staff are onsite in each facility to offer freshly cooked, nutritious, appetizing, in-season meal options. Our residents truly enjoy three wholesome meals each day and delicious snacks during the morning, afternoon, and evening.

 

 

Activities

Our residents are offered exciting opportunities to visit local museums, take scenic rides throughout beautiful Saratoga County, excursions to the Saratoga Race Track as well as nearby lakes, parks and musical venues. Every resident can enjoy daily exercises, art classes, games, as well as a full service beauty salon, library, and large chapel in each building. We offer a full calendar of social and recreational activities that keep residents challenged and active.

 

 

Transportation

 Transportation

Transportation to and from doctor’s appointments are provided Monday-Friday. A member of our care staff will accompany each resident to the doctor. The aide stays with the resident (if needed) and communicates with the doctor to assure all medical issues are addressed.

 

 

Care Management

Care Management

Our case manager works hand in hand with our nurses as well as community providers, to manage the medical, social, physical and mental needs of each individual resident to ensure a positive quality of life and maintain a level of independence. The Case Manager is a primary source of communication with the resident’s Primary Care Physician in regard to the medical well-being of the resident.

 

 

Medication Management

Our Licensed Nurses are on site and provide medication assistance to our residents 24 hours a day. This includes ordering and storage and assistance with administering medications.

 

 

Laundry and Housekeeping

 Laundry and Housekeeping

As well as daily bed making and straightening of rooms, full housekeeping services are provided twice weekly. Laundry services are provided one to two times weekly or more as needed. Resident’s clothing does not have to be labeled with our efficient laundry system.
