All five of our communities are licensed by the NY State Department of Health and provide a range of services, including a continuum of care, allowing residents to “age in place.” Many seniors require additional care as they grow older and are no longer able to care for themselves in an independent living environment, such as their home or an independent living community.
Independent Living With A Helping Hand
Home of the Good Shepherd Assisted Living provides a level of care above independent living, but residents enjoy warm and inviting private modern suites, three freshly prepared meals created with in-season ingredients, laundry and housekeeping, exciting social activities such as scenic rides, entertainment or lunch outings, a full-service beauty/barbershop, and transportation to doctor’s appointments. In addition, our Assisted Living also offers: medication management, assistance with ADL’s and total assistance with ADL’s including bathing and showering.
Upon admission, a personal care plan is developed for each resident and integrated within care management services. The personal care plan reflects an individual resident’s needs and routines prior to admission and is based on discussions with the resident, family members, physicians, and other healthcare providers involved in the well-being of the resident. The Care Plan is routinely updated based on nursing assessments done by onsite staff as the resident ages in place.
