As an LPN Supervisor, you will play a vital role in supporting our residents by:

Provide oversight and guidance to a team of 10–12 CNA, HHA, and RA staff delivering care to elderly residents with physical and/or cognitive disabilities
Administer medications in accordance with facility protocols and state regulations
Supervise and support staff in delivering high-quality care and assistance with activities of daily living (ADLs)
Monitor and document the overall health and well-being of residents
Collaborate with Directors and Administration to ensure compliance with NYSDOH regulations and guidelines
Assist with Case Management duties as scheduled to support individualized resident care plans

Job Type:

All shifts, full-time, part-time, per-diem

Hours:

Must be able to work every other weekend. Per-diem will need to work at least 1 weekend/month.

Days:

Entry Level LPN Starting at $29.00/hour for Day shift, additional pay based upon experience, Max $30.25
LPN 5+ years experience Day shift – Starting at $30.00/hour, additional pay based upon experience, Max $32.50

Evenings:

Entry Level LPN Starting at $30.00/hour for Evening shift, additional pay based upon experience, Max $31.25
LPN 5+ years experience Evening shift – Starting at $31.00/hour, additional pay based upon experience, Max $33.50

Overnights:

Entry Level LPN Starting at $31.00/hour for Overnight shift, additional pay based upon experience, Max $32.25
LPN 5+ years experience Overnight shift – Starting at $32.00/hour, additional pay based upon experience, Max $34.50

Shift Differential:

Evenings $5.00
Overnights $3.00

Requirements:

Active New York State Licensed Practical Nurse (LPN) license
Strong supervisory and leadership skills to manage and motivate care staff
Knowledge of NYSDOH regulations and healthcare best practices
Clinical experience with elderly populations, including medication administration, wound care, and ADL support
Ability to respond effectively to resident needs and emergencies
Physical capability to stand, walk, assist residents, and perform nursing care tasks over extended periods
Reliable, compassionate, and committed to high standards of care
